Refer Your Patient

The CU Fertility Preservation and Reproductive Late Effects (FPRLE) Program is designed to address gaps in care and to improve the fertility and reproductive health of patients receiving chemotherapy, radiation, and/or surgical procedures that may impact fertility. The program encompasses fertility preservation care for newly diagnosed and relapsed patients, and also reproductive healthcare for patients both at diagnosis and in cancer survivorship.

For fertility preservation, we accept referrals and consults for females who are pre-pubertal through age 42 and males who are pre-pubertal through age 50. These patients have received or will undergo chemotherapy, radiation therapy, and/or surgical procedures of the reproductive tract for treatment of their condition. Reproductive healthcare needs addressed include a discussion regarding the impact of medical treatment on fertility, a review of available fertility preservation options, contraceptive management, menstrual suppression, and applicable diagnostic lab testing.

Reproductive late effects include adverse gynecologic and urologic outcomes of treatment. Care is provided to lessen the severity of these potential adverse effects. We accept referrals and consults for male and female patients (as appropriate) of all ages for:

  • Ovarian or testicular failure
  • Genital graft-versus-host disease in males and females
  • Direct radiation effects to the breast and pelvic organs
  • Sexual dysfunction during cancer treatment and in survivorship
  • Sexual dysfunction during cancer treatment and in survivorship
  • Pregnancy considerations in survivorship
  • HPV-related dysplasia and other health maintenance needs

For new FPRLE Program Referrals and Consults

  1. Place Referral or Consult order in EPIC
  2. Page the FPRLE Team:
    • For University of Colorado Hospital: Pager is listed in AMiON within Reproductive Endocrinology Department.
    • For Children's Hospital Colorado: Pager is listed in AMCOM within Pediatric and Adolescent Gynecology Department.
    The FPRLE team will promptly return calls.
  3. Outpatient: AMB Referral to Fertility Preservation/Reproductive Late Effects > Priority: Urgent
    • Reason for Referral: List diagnosis and planned treatment regimen
    • Fertility Preservation Referrals: Patients receiving chemotherapy or radiation are to be seen prior to treatment initiation.
    • Reproductive Late Effects Referrals: Referral can be placed at any point during patient’s treatment regimen, or in survivorship.
    Outpatient referrals will occur in a timely fashion after discussion with the requesting provider.
  4. Inpatient: IP Consult to Fertility Preservation/Reproductive Late Effects > Priority: Urgent
    • Reason for Consult: List diagnosis and planned treatment regimen.
    • Fertility Preservation Consults: Patients receiving chemotherapy or radiation are to be seen prior to treatment initiation.
    Inpatient consults occur where the patient is located and within 24 hours of consult request.
